Event Request Form - Private Reservations

Eastview church members may use the buildings/rooms on the Eastview Campus for approved personal/private events free of charge. The pastoral staff must approve all campus events & activities. Members are welcome to make a donation to assist in covering expenses incurred with use, but it is not required.

You are welcome to use the pots, pans, and serving dishes. Disposable pantry items purchased by Eastview are not for personal use. This includes pantry cooking supplies such as cooking oil, seasonings, drinks, aluminum pans, ziplock bags, etc.
Please note: The ice machines must be turned on in advance to produce ice. If you prefer, you may bring the ice needed for your event.

Please describe the desired room set-up. Quantities of tables & chairs available vary by building.
Tables & chairs can not be moved from one building to another.

60 in round tables
black folding chairs (Family Life Center)
metal folding chairs (Children's Center)

Your Responsibilities

I understand I am expected to be a good steward of the areas of the Eastview UPC campus used for my event.

The use of alcohol and tobacco is prohibited in all areas of the campus.
Children must be supervised at all times.
Food and drinks are to be confined to the tiled areas of the campus.
Food and drinks are not allowed on carpeted areas or the gym unless prior approval is given.
Taping, mounting, or displaying items on any painted surface is not allowed.

I understand I am responsible for the set-up, clean-up, and return to normal set up of the facility or area used.
I will remove all items brought in for use at the event immediately after the event unless given prior permission. (decorations, props, food, etc.)
I will return all items used to their proper storage locations.
I will remove all trash from the space used and take it to the dumpster.
I will wipe down tables & countertops after use.
I will spot clean any sticky spills with hot water only.
I will sweep the floor, and I will vacuum if it is needed.
I will turn off Heat/AC if I turned it on.
I will ensure the building is secure by locking all doors and turning off lights.
I understand I am not responsible for mopping floors or detailed cleaning.

A chaperone must be present at all times.
Chaperones must be church members age 21 or older.
Church members are responsible for guests complying with the rules.
No food or drink is allowed in the gym play area.
No foul language.
No unsportsmanlike conduct.
Make sure the gym is clean after play.
Put all equipment away after use.
Turn all lights & AC/Heat units off.
**Tennis shoes only in the gym play area.**
Fellowship Hall/Kitchen is off-limits unless given prior approval.
Items such as tables, chairs, kitchen items, and gym equipment are not be removed from the building.

Gym Dress Code for Males:
No shorts of any kind.
No cut-off shirts.
No sleeveless shirts unless worn over a shirt with sleeves.

Gym Dress Code for Females:
Skirts only - length to the knee
No sleeveless or cap-sleeve tops.
No midriff shirts.

Church events always take priority over private events.
Private events taking place on church property must be in compliance with Eastview UPC's beliefs, policies, and statement of faith.
While we will do our best to ensure your event is able to happen as you desire, we simply cannot foresee some events such as funerals or other short-notice situations that could necessitate the cancellation of your event.

All use of Eastview facilities requires Pastoral approval. The church office will contact you regarding approval or denial of your event request.
